…Okay, so there IS a connection to Lucifer in that memory. Which was that he sliced into Katou's shoulder SO BAD that he couldn't repair the damage, and his arm was basically rotting off. So he'd gotten it replaced with this arm. He doesn't have the memory of getting the replacement yet, but he does actually remember getting his shoulder sliced. ANYHOW, that's just background information for this memory.
You see, the important thing about Katou's shape-shifting is that it takes concentration. He looks like himself because he IMAGINES looking like himself, which is why he still has things like his tattoo, track marks, etc. Because those are so much a part of how he pictures himself, that it would take MORE effort to not have them. In canon— and this is another memory that he has, but hasn't spoken about— he gets amnesia at one point, and it's stressed that before he finds a form to hold onto, HE HAD NO FACE. Nothing. He was just... basically, physically speaking, a moving doll with a spirit inside.
This wound that he has is so bad that between the pain of it and the imperfect arm graft, he's having to concentrate harder on who he is to maintain his form. THE PAIN IS SO BAD, he's starting to lose his memories, piece by piece, and it's slowly "killing" him— a death of the soul, so to speak. He'd taken to cutting at himself, to distract from THAT pain and remind himself of what's important to him, that he needs to hold on, THAT HE NEEDS TO REMEMBER. He promised Setsuna, he promised Kira. Because he hates the person he was, wants to be worth more. He wants to keep going until the very end, he wants to be MORE than just a delinquent that nobody cared about. Possibly, Uriel could've fixed him, could've helped him recover, but that would've taken him out of the fight, and that was completely unacceptable to him. He had to keep going… for his own sake, as much as anyone else's. Even if it was killing him. '-'
